- Download the Camp Resilience Applicant Verification form below, give it to your VA therapist or physician and ask him/her to to complete it, and either e-mail it to info@camp-resilience.org; fax it to us at (855) 335-8252; or mail it to: Camp Resilience, PO Box 7384, Gilford, NH 03247-7384.
